Documentation is one of the most powerful ways recreational therapists communicate their clinical role—yet it’s often where that role gets lost.
Listing activities without therapeutic intent doesn’t reflect the clinical reasoning behind RT services. Documentation that highlights assessment findings, goals, interventions, and outcomes tells the real story of treatment.
Clear, outcome-focused documentation supports continuity of care, strengthens interdisciplinary communication, and accurately represents the value of recreational therapy.
